Though I practice shooting with ear protection, I am concerned about the possible disorientating effects of loud gunfire in a real situation, Should I occasionally practice a few shots without protection to be better prepared for those incidents ?

I would say no. Audio exclusion is a well documented phenomenon in cases where the limbic system has been activated and would serve to prevent disorientation.
